All-on-X vs. All-on-4: What the Number Actually Means

The names All-on-4 and All-on-X describe one idea: a complete arch of fixed teeth attached to implants, replacing a full set of failing or missing teeth, or a denture. The difference is the number. All-on-4 commits to four implants per arch. All-on-X leaves the number open until the jaw has been measured. Everything else — the bridge, the healing, the cleaning — is the same kind of treatment.

What All-on-4 means

Four implants support one full arch. The two at the front are usually placed upright; the two at the back are often tilted, so they can use the bone that is there and avoid the sinus in the upper jaw or the nerve in the lower. For the right patient, that angle can avoid grafting and shorten treatment. It is a well-established approach, and for many people four is enough.

What All-on-X means

The X is a placeholder. The plan starts from your jaw rather than from a number: the implants may be four, five, six or occasionally more, placed where they give the arch the most dependable support. All-on-X is not a different kind of treatment; it is a refusal to decide the implant count before the scan.

Why the number changes

  • Bone quality varies. Bone can be dense in one part of the jaw and soft or thin in another, and the upper jaw's bone is often softer than the lower's.
  • Some bites are heavy. Grinding, clenching or a history of breaking teeth puts more force on the arch, and more implants share it.
  • Arches differ in size. A broad arch or a long bridge may be better supported by more than four.
  • Both arches at once. When upper and lower arches are replaced together, they are planned as one system, and the implant layout on each can change to make the bite work.

More implants are not automatically better. Each one is more surgery and more cost, and four well-placed implants can do very well. The goal is the right number in the right places, not the most.

Side by side

  • Implants per arch. All-on-4: four. All-on-X: set by the plan — often four to six.
  • Grafting. Both aim to use existing bone; tilted back implants often let All-on-4 avoid grafting. Some All-on-X plans add implants instead of grafting.
  • Cost. Fewer implants can lower part of the cost, but the implants are only one line in a full-arch plan.
  • Best suited to. All-on-4: a healthy, moderate bite with enough bone in the front of the jaw. All-on-X: uneven bone, heavy bite forces, a broad arch, or both arches at once.

The teeth right after surgery

In many full-arch cases a temporary fixed arch is attached at surgery or within days, so you don't leave without teeth. Whether that is possible depends on how firmly the implants are seated when they go in, so it is decided at surgery rather than promised in advance. The temporary is not the finished set: it protects the implants while they fuse, and it comes with a soft diet for months — what to eat after implant surgery covers the stages. What the bridge is made of

The final bridge is its own decision. Common choices are acrylic teeth on a reinforced frame, or zirconia — each with trade-offs in weight, how easily it can be repaired, and how it resists wear, chipping and staining. Ask what the temporary and the final bridge are made of, and whether both are in the quoted plan.

Cost: look past the implant count

It is natural to assume four implants cost less than six, and the implants themselves often do. But a full-arch plan also includes extractions, imaging, sedation, the temporary arch, the final bridge, lab work, follow-up visits and any grafting or gum treatment, so the implant count rarely decides the total. Choosing fewer implants just to lower the first number can be a false economy if the support isn't right for your jaw. Who is a candidate

People missing most or all of the teeth in an arch, whose remaining teeth can't be predictably saved, or whose denture moves and rubs. Uncontrolled gum infection, unmanaged medical conditions and heavy smoking affect healing and may need attention first — implants if you smoke, have diabetes, or are older covers those. Looking after it

A full arch is fixed, not maintenance-free. It is cleaned in place every day, including underneath, with the brushes or water flosser the office shows you, and checked at regular professional visits. Ask for the reasoning, not just the number

Whether the recommendation is four implants or six, the question is the same: what about my bone and my bite led you to that number, and what would change it? A plan that can answer that has been built for your jaw rather than picked from a menu.
